Care Route Logistics LLC provides:

• Mobile specimen delivery
• DOT drug and alcohol testing support
• Rapid testing services
• Courier and transport services
• Chain-of-custody specimen transport

Services are available in Albuquerque and surrounding areas, including 24/7 service availability when scheduled.

2 Payment Terms

Payment is due upon receipt unless prior billing arrangements or contracts are established.

Accepted payment methods may include:

• Cash
• Business checks
• Square invoices
• Electronic payments

Late payments may result in service interruption.

3 Mileage & Travel Fees

Standard service pricing includes travel within a 50-mile radius of Albuquerque, NM.

Additional mileage beyond 50 miles will be billed at:

Emergency or after-hours requests may include additional fees.

4 Client Responsibilities

Clients are responsible for:

• Providing accurate pickup and delivery information
• Proper labeling and packaging of specimens
• Ensuring authorized personnel are available for release and receipt of specimens
• Compliance with applicable testing regulations

5 Limitation of Liability

Care Route Logistics LLC is not responsible for delays caused by:

• Weather conditions
• Road closures
• Facility delays
• Incorrect client information
• Circumstances outside reasonable operational control

6 Confidentiality

All services are conducted with strict confidentiality and professionalism. Client information and medical-related materials are handled according to HIPAA-aware procedures.

7 Service Availability

24/7 services are subject to scheduling and availability.

Same-day and emergency services are offered based on route capacity.

8 Cancellation Policy

Clients are encouraged to provide at least 2 hours notice for cancellations.

Repeated cancellations may affect future scheduling priority.
